The primary purpose of your job position is to maintain the resident census of Fairhaven Healthcare Center by developing and maintaining a network of customer relationships with hospitals, nursing homes, insurers, and community organizations. In addition, manages the admissions process from resident screening to processing of all required admission paperwork.
DELEGATION OF AUTHORITY
As the Assistant Admissions Liaison you are delegated the responsibility and accountability necessary for carrying out your assigned duties, and will report directly to the Director of Admissions
ESSENTIAL JOB FUNCTIONS: RESPONSIBILITIES AND STANDARDS
Identifies marketing opportunities By Identifying consumer needs; defining market, competitor’s share, and competitors advantages and weaknesses; forecasting projected admissions/business.
Coordinate multiple aspects of converting a new patient referral to a new patient admission.
Interfacing consistently and professionally with Licensed Nursing Case Managers from multiple healthcare insurers across the country.
Acquiring prior authorizations from Insurers.
Sustains rapport with key businesses By Making periodic visits; exploring specific needs; anticipating new opportunities; promptly responding to requests for screening of residents for potential admission either by phone or traveling to the referral’s site.
Assist with the planning, developing, implementing, and evaluating, advertising, and trade promotion programs.
Identifies resident needs By Establishing personal rapport with potential and actual residents and other persons in a position to understand service requirements, providing support, education and information to residents and their families through the admissions process.
Maintains admissions flow By Carefully evaluating each resident (pre-admission) to ensure their needs can be met at The Fairhaven Healthcare Center and which maximize revenue yet minimize financial risk; coordinating and providing pertinent clinical and financial information to the healthcare team; ensuring a smooth admission transition for the resident and family; authoring the final acceptance of residents; collaborating with the Director of Nursing, Case Manager, Director of Rehab Services, Social Services, Therapeutic Recreation, and Dietary department personnel.
Maintains the stability and reputation of the health care facility By Complying with accreditation requirements; projecting a personal and professional image when
Dealing with people external to the facility.
Prepares reports By Collecting, analyzing, and presenting information.
Maintains residents’ confidentiality and protects operations By Keeping information confidential; following release of information protocols; following state laws and federal regulations regarding the right to access and release of health information; ensuring that clinical documentation is informative, descriptive, and timely.
Assists the Admissions Department By Planning, developing, organizing, implementing, evaluating, and directing various programs and activities; developing appropriate admission forms and marketing materials.
Nursing degree from an approved nursing college; or college degree in Social Services related fields.
Current Massachusetts Nursing or Licensed Social Work License
Two years or greater experience in long term care management
Two years or greater in sales or marketing in healthcare or health insurance industry
Case management experience preferred
