To provide customer service and case management services to families, children, and youth served by Spaulding Academy & Family Services. To value the role of families in their youth’s treatment and to ensure family voice is supported in their child’s treatment. Permanency Specialists will need to be able to work in a flexible way and be able to multitask as needed for the needs of the parents and the rest of the team. Permanency believes in a “growth set model” where growth of a case is supported by a team of staff. The role of Permanency is to navigate the youth’s treatment from admission to post discharge. To enhance transition and after-care services by supporting families and youth through an individualized community-based, trauma informed, culturally sensitive supports during the onset of admission, throughout placement and for at least 6 months post discharge as indicated in order for which builds on an extension of the treatment and promotes safety and well-being by identifying and securing resources for the individual/ family in order to maintain the child in a family. To enhance and support incident debriefing with students and their families to be inclusive of family voice. Assisting, enhancing, and supporting families through the guardianship process.
