Job Title: Clinical Social Worker – LICSW
Location: Hyannis, MA
Position Overview
"Navitas Healthcare, LLC" is is seeking a Clinical Social Worker (LICSW) to provide comprehensive psychosocial assessment, counseling, discharge planning, and care coordination services. The role supports patients and families through illness, recovery, and transition of care in collaboration with interdisciplinary teams and community resources.
Key Responsibilities
- Conduct psychosocial assessments of patients and families
- Develop individualized discharge and care plans
- Provide counseling, emotional support, and crisis intervention
- Coordinate with healthcare teams and community agencies
- Support safe transitions to post-acute or community care
- Maintain accurate documentation and compliance standards
- Participate in quality improvement and professional development activities
Required Qualifications
- Master’s Degree in Social Work (MSW)
- Active LICSW (Massachusetts)
- Strong clinical assessment and crisis intervention skills
- Excellent communication and organizational abilities
- Knowledge of Social Work Code of Ethics
Additional Requirements
- CPI / MAB certification within 1 month of hire (annual renewal required)
- Acute care hospital experience preferred
- Ability to manage multiple cases in a fast-paced environment
