Job DescriptionPhysical Therapist – Long Term Care/RehabilitationSeeking a dedicated Physical Therapist to evaluate, develop, and implement individualized treatment plans for patients with physical impairments in a Long Term Care/Rehabilitation setting. Responsibilities include therapeutic interventions, wound care, patient and caregiver education, progress monitoring, documentation, discharge planning, and collaboration with the interdisciplinary care team. The role also includes supervising Physical Therapist Assistants and adhering to all clinical and regulatory standards.
Requirements:
- Active Massachusetts Physical Therapist license
- Minimum 1 year of PT experience (new graduates with Level II clinical experience are welcome to apply)
- Long-term care or Rehabilitation experience required
- Strong communication, documentation, and patient care skills
