In this position, you will provide exceptional, patient-focused rehabilitation services across various care environments. Your clinical skills will be essential in assessing patients, crafting personalized treatment plans, and applying evidence-based physical therapy techniques that foster optimal recovery, functional independence, and overall health.
As a Physical Therapist, you will conduct thorough evaluations to pinpoint impairments, functional challenges, and patient aspirations. You will then create and modify care plans tailored to each individual’s needs. Your hands-on treatment will encompass therapeutic exercises, manual therapy, gait and balance training, and functional mobility training, all while closely tracking progress and adjusting interventions as necessary. Additionally, you will educate patients, their families, and caregivers about conditions, home exercise routines, and strategies to enhance safety, mobility, and quality of life.
Collaboration is key in our supportive, team-oriented environment that prioritizes integrity and innovation. You will work alongside occupational and speech therapists, nurses, physicians, and other healthcare professionals to contribute to coordinated care plans. Participation in case conferences and interdisciplinary meetings will be vital to improving patient outcomes. Accurate and timely documentation in the electronic medical record will be a crucial aspect of your daily responsibilities, ensuring continuity and quality in care.
