Registered Nurse (RN) - Homecare & HospiceLocation: North Conway, NH
Employment Type: Full-Time / Part-Time | 24-40 hours/week
Setting: In-Home Care / Home Health & Hospice
Compensation: $37.02-$50.09/hour, based on education and experience
Key Responsibilities

Conduct patient assessments and develop individualized care plans.

Provide skilled nursing care, including wound care, medication administration, symptom management, and patient education.

Coordinate care with physicians and other healthcare professionals.

Support patients and families with education and emotional guidance.
-
Maintain accurate documentation in the EMR.
-
Help promote quality outcomes and reduce avoidable hospitalizations.
Schedule
-
Day Shift: 8:00 AM - 4:30 PM
-
Hours: 24-40 hours/week
-
Weekend Call: Approximately once per month
-
Shift Differential: Available for applicable on-call, weekend, and night coverage.

Travel to patients' homes is required.
Qualifications
-
Active RN license in New Hampshire and Maine
-
New graduates are welcome
-
Home care, long-term care, and/or hospice experience preferred

Strong assessment and critical-thinking skills
-
Ability to work independently in a home-based environment

Comfortable using EMR/mobile documentation systems
-
Valid driver's license and reliable transportation required
Patient Population
-
Various age groups, with mostly adult patients
-
Approximately 5 patient visits per day, depending on patient needs and scheduling
