Hospice RN / Home Health RN / Wound Care RN
\n Location: Newport, VT 05855
\n Shift: Monday–Friday, 8:00 AM–5:00 PM
\n Weekends: Some weekend coverage required
\n Call: Some call may be scheduled, but only on an emergency basis
\n Requirements:
\n OASIS (Outcome and Assessment Information Set) experience required
\n Home Health and Hospice experience required
\n Wound Care experience preferred/required as applicable
\n Experience traveling from patient to patient for home visits
\n Must be able to manage and adjust to changes in the schedule
\n Valid driver’s license in good standing required
\n BLS certification required
\n EMR: HealthWyse (Careficient)
\n Outpatient care experience
\n Facility conducts its own background check
\n
