About the Role
Lydia's House of Hope is seeking a highly organized, compassionate, and dependable Part-Time Case Manager to support women and children participating in our residential transitional program.
Our Case Managers do much more than provide referrals. They walk alongside residents as they work toward stability, independence, and long-term success. This position requires someone who can build trusting relationships while maintaining healthy professional boundaries, identify needs, coordinate services, solve problems, and consistently follow responsibilities through to completion.
The right candidate will be compassionate and accountable—someone who notices what needs to be done, asks questions when something is unclear, takes initiative when a barrier arises, and follows through until the matter is resolved.
Key Responsibilities
- Develop and maintain individualized case plans based on each resident's needs, goals, and progress.
- Meet regularly with residents to review goals, challenges, appointments, services, and next steps.
- Coordinate medical, mental health, educational, childcare, transportation, employment, housing, benefits, and community-resource needs as appropriate.
- Maintain accurate resident schedules, appointments, deadlines, documentation, and case notes.
- Follow assigned responsibilities from beginning to completion, including confirming appointments, transportation, required documentation, referrals, and other arrangements.
- Verify information before communicating that a task, referral, appointment, or arrangement is complete.
- Recognize barriers and independently determine appropriate next steps; seek supervisory assistance promptly when additional guidance is needed.
- Follow up with schools, daycare providers, medical providers, agencies, and community partners to ensure required information and services have actually been received.
- Anticipate upcoming resident needs and deadlines rather than waiting for a problem or missed appointment to occur.
- Communicate clearly and accurately with residents, staff, supervisors, and outside providers.
- Advocate appropriately for residents while encouraging personal responsibility, growth, and increasing independence.
- Maintain professional boundaries and apply program expectations consistently.
- Collaborate closely with the House Supervisor, Executive Director, other Case Managers, and residential staff.
- Respond appropriately to resident crises and communicate concerns promptly according to program procedures.
- Maintain confidentiality and complete documentation accurately and on time.
- Participate in staff meetings, supervision, training, and professional development.
- Perform other case-management responsibilities necessary to support resident safety, stability, and successful program participation.

Qualifications
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred; relevant experience may be considered.
- Previous experience in case management, residential services, social services, behavioral health, or a related field preferred.
- Experience working with women, children, families, homelessness, trauma, recovery, or other vulnerable populations is beneficial.
- Knowledge of community resources and social-service systems is preferred.
- Strong written, verbal, organizational, and computer skills.
- Ability to maintain confidential and accurate resident records.
- Valid driver's license, safe driving record, and reliable transportation required.
- Ability to meet the scheduling and transportation needs associated with a residential program.

What Success Looks Like in This PositionA successful Case Manager doesn't simply make a referral or begin a task. They follow through.When an obstacle arises, they ask: What is the next step? Who needs to be contacted? What information is missing? Has this actually been resolved? They communicate accurately, document appropriately, ask for help when needed, and make sure important resident needs do not fall through the cracks.
Most importantly, they understand that behind every appointment, school form, transportation arrangement, referral, and deadline is a woman or child depending on our team to do what we said we would do.
