Intake Service Coordinator – Adult Day Health

Location: Attleboro, MA (Primary) | Multisite including Brockton
Schedule: Full-Time | Monday–Friday | Day Shift
Pay: $23–$25/hour + Milage Reimbursement

The Intake Service Coordinator manages the referral and intake process for our Adult Day Health Centers, supporting multiple sites with a primary focus in Attleboro and Brockton. This role plays a key part in strengthening professional relationships throughout the community and ensuring a smooth admissions process.

Key Responsibilities

- \n
- Manage and process all incoming referrals
- Develop and maintain strong professional relationships with referral sources, community partners, and internal teams
- Serve as a primary point of contact for referral inquiries and service coordination
- Coordinate with clinical and operations teams to facilitate appropriate placements
- Oversee the full intake process from referral through admission
- Collect and verify demographic, insurance, and funding information
- Submit authorizations and ensure payer verification
- Utilize working knowledge of MMIS (MassHealth/Medicaid platform)
- Collaborate with ASAP agencies and Managed Care Organizations
- Maintain accurate documentation and ensure compliance with regulatory standards
- Identify and resolve documentation or funding issues
- Support multisite operations as needed

Qualifications
\n
- \n
- Bachelor’s Degree in Human Services or related field preferred
- Minimum 2 years of relevant experience preferred (equivalent experience considered)
- Working knowledge of MMIS (MassHealth), Medicaid, payer verification, and authorizations
- Office management experience strongly preferred
- Strong relationship-building and customer service skills
- Highly organized, detail-oriented, and able to multitask
- Professional, approachable, and comfortable engaging with community partners
