Location: Attleboro, MA (Primary) | Multisite including Brockton
Schedule: Full-Time | Monday–Friday | Day Shift
Pay: $23–$25/hour + Milage Reimbursement

The Intake Service Coordinator manages the referral and intake process for our Adult Day Health Centers, supporting multiple sites with a primary focus in Attleboro and Brockton. This role plays a key part in strengthening professional relationships throughout the community and ensuring a smooth admissions process.

Key Responsibilities

\n

    \n
  • Manage and process all incoming referrals
  • \n

  • Develop and maintain strong professional relationships with referral sources, community partners, and internal teams
  • \n

  • Serve as a primary point of contact for referral inquiries and service coordination
  • \n

  • Coordinate with clinical and operations teams to facilitate appropriate placements
  • \n

  • Oversee the full intake process from referral through admission
  • \n

  • Collect and verify demographic, insurance, and funding information
  • \n

  • Submit authorizations and ensure payer verification
  • \n

  • Utilize working knowledge of MMIS (MassHealth/Medicaid platform)
  • \n

  • Collaborate with ASAP agencies and Managed Care Organizations
  • \n

  • Maintain accurate documentation and ensure compliance with regulatory standards
  • \n

  • Identify and resolve documentation or funding issues
  • \n

  • Support multisite operations as needed
  • \n

Qualifications

\n

    \n
  • Bachelor’s Degree in Human Services or related field preferred
  • \n

  • Minimum 2 years of relevant experience preferred (equivalent experience considered)
  • \n

  • Working knowledge of MMIS (MassHealth), Medicaid, payer verification, and authorizations
  • \n

  • Office management experience strongly preferred
  • \n

  • Strong relationship-building and customer service skills
  • \n

  • Highly organized, detail-oriented, and able to multitask
  • \n

  • Professional, approachable, and comfortable engaging with community partners
  • \n
