We’re seeking an experienced Central Admissions Coordinator to lead hospital referral development and manage patient transitions into our skilled nursing facilities. The ideal candidate has direct experience in long-term care admissions or hospital discharge coordination. They should confident in building relationships with case managers, physicians, and referral sources. Job duties:

· Demonstrate growth in hospital referrals, as reflected in an increase of overall occupancy and quality mix in assigned long-term care facilities.

· Help create an atmosphere of warmth and calm within the facilities as well as a culture that actively supports company’s mission, vision and values.

· Build strong relationships with hospitals, doctors, case managers, and insurance companies.

· Track daily bed availability and share updates with referral sources.

· Manage referrals by honoring patient requests or placing them in appropriate facilities based on needs and availability.

· Work with nursing and business staff to ensure smooth patient admissions and transitions.

· Gather and share accurate patient and discharge information with the care team.

· Follow internal admission procedures and involve clinical leaders when needed.

· Monitor hospital readmissions and follow up on patient return plans.

· Attend monthly meetings with facility administrators to review performance and set new goals.

· Keep patient information private and follow all HIPAA guidelines.

· Help resolve any issues between hospitals and the facility to ensure quality service.

Requirements:

  • Two+ years of experience interacting and working with hospital-based clinicians (case managers, nurses, discharge planners and social workers). Must have the clinical knowledge to effectively screen potential new residents
  • Direct experience with post-acute admissions or hospital discharge coordination
  • Knowledge of Medicare, Medicaid, managed care, and insurance authorizations
  • Comfortable working independently across multiple sites
