This position is set within a remote environment, allowing for flexibility in how support is delivered to our clients at the Detox Center. Despite the remote nature, collaboration remains a cornerstone of our practice; you will work closely with interdisciplinary teams including nurses, caregivers, and other healthcare professionals to ensure the most effective care is provided.
Key Responsibilities\n

- Carry out thorough psychosocial assessments for the elderly, factoring in emotional, mental, and social determinants relevant to their well-being.
- Collaborate with clients and their families to craft personalized care plans tailored to meet individual goals and preferences.
- Provide counseling and emotional support to clients, guiding them through life changes and minimizing challenges.
- Act as an advocate for clients' rights, ensuring they have seamless access to community resources and necessary support services.
- Engage with interdisciplinary teams to facilitate comprehensive care, participating in case discussions that focus on the health and well-being of clients.
- Conduct regular virtual visits to assess client conditions and ensure their living environments are safe and supportive.
- Serve as a mediator and resource for families, helping to resolve conflicts and improve communication among all involved in the client’s care.

- Maintain precise and timely documentation of assessments, care plans, progress notes, and significant interactions within our electronic health record system.
