Hospice Chaplain
Location: New Hampshire and Maine Seacoast area
Department: Hospice Care
Status: Part Time 24 hours
Reports to: Holistic Discipline Manager

As a Hospice Chaplain, you will play a vital role in supporting patients and families during one of life's most sacred transitions. You'll be part of a dedicated Interdisciplinary Group (IDG) delivering whole-person care that honors every individual's values, beliefs, and legacy.
What You'll Do:
Spiritual Assessment & Patient Support

Complete spiritual assessments within five days of hospice admission (as requested).

Provide individualized spiritual and emotional support aligned with the beliefs of patients and families.

Participate in care conferences to assess and address spiritual needs.

Coordinate with community clergy for continued support as appropriate.
Team Collaboration & Documentation

Actively contribute to bi-weekly IDG meetings.

Offer staff support through debriefings and 1:1 grief counseling.

Document all visits in a timely and accurate manner using agency systems.
Bereavement & Community Services
-
Assist with or lead memorial services, including annual remembrances and family-requested events.

Help coordinate and maintain a clergy resource network.

Support bereavement program functions as needed.
Education & Professional Development

Educate staff and community clergy on hospice philosophy and spiritual care principles.

Commit to continued learning, including hospice-specific trainings (18 hours in the first year, 8 hours annually thereafter).
Productivity Expectations:

Maintain 3–4 patient visits per 8-hour day

Documentation sync'd by midnight 95% of the time
-
70% or greater in-home documentation rate
Environment & Tools:

Primarily field-based role (90% outside, 10% inside)

Daily use of laptop/computer for documentation

Who You Are:

Compassionate and centered, with strong listening and communication skills

Experienced in providing spiritual care, ideally in hospice or palliative settings
-
Able to work independently and collaboratively within a multidisciplinary team

Comfortable supporting diverse beliefs, cultures, and end-of-life rituals

Familiar with EMR systems and clinical documentation expectations
Qualifications:

Ordination or equivalent credentials in one's faith tradition

End-of-life care experience strongly preferred

Valid driver's license and reliable transportation

Ability to meet all required orientation and training standards
