Responsibilities:

Provide emotional support and guidance to residents and their families, addressing their mental health and psychosocial needs

Conduct psychosocial assessments and develop individualized care plans to meet residents' social and emotional needs

Facilitate communication between residents, families, and healthcare teams to ensure comprehensive care

Develop and implement interventions to address residents' mental health and cognitive issues

Collaborate with interdisciplinary teams to ensure resident-centered care and effective discharge planning

Maintain accurate records and documentation of social work services and interventions
Requirements:
-
ASSOCIATES degree in Social Work or related field

1 year of experience in social work

Licensure as a Social Worker in the state of Vermont

Strong communication and interpersonal skills

Ability to prioritize and manage multiple tasks and responsibilities
