Key Responsibilities\n

- Evaluate patients and develop individualized physical therapy treatment plans
- Implement therapeutic interventions to improve patient mobility, strength, and function
- Monitor and document patient progress throughout the course of treatment
- Collaborate with interdisciplinary care team members to coordinate patient care
- Maintain valid state or federal issued photo identification and wear a photo badge at all times while on site

Required Qualifications\n

- Active Physical Therapist (PT) license in the state of New Hampshire or compact licensure eligibility
- Graduate of an accredited Physical Therapy program
- Minimum 35-hour weekly availability to meet guaranteed hours requirement
- Valid state or federal issued photo identification required upon arrival at facility
