Licensed Practical Nurse, Long-Term Care travel assignment in Middlebury, Vermont. Start date 04/28/2026 for a multi-week assignment with guaranteed 40 hours per week. Responsibilities
- Provide direct nursing care to residents in a long-term care setting under supervision of an RN or physician.
- Monitor residents, measure and document vital signs, and report changes promptly to the care team.
- Administer prescribed medications and treatments in accordance with orders and facility policy.
- Perform basic wound care, injections (per scope), and routine clinical tasks as directed.
- Assist residents with activities of daily living (ADLs), mobility, nutrition, hygiene, and safety-focused care.
- Maintain accurate charting and documentation; communicate effectively with interdisciplinary team to update care plans.
Requirements
- Active Licensed Practical Nurse license in Vermont
- Current Basic Life Support (BLS) certification
- Graduate of an approved LPN program
- Prior long-term care or assisted living experience preferred
- Ability to work effectively in a long-term care/assisted living setting
- Strong communication and documentation skills
