Position Summary
The Re-Housing Specialist will provide case management and housing support to families in shelter. This role conducts assessments, develops housing plans, andassistsparticipants in securing andmaintainingstable housing. This is a hands-on, in-person role withmeaningfulimpact on the lives of families in our community.
WhatYoullDo
- Conduct intakes, assessments, and goal planning with shelter participants
- Provide ongoing case management through individual and group support
- Maintainaccurateandtimelydocumentation inEOHLCprogram systems
- Assistparticipants with housing search, applications, and placement
- Develop andmaintainlandlord relationships; act as liaison as needed
- Connect participants to employment, education, and supportive services
- Participate in case conferencing with internal and external providers
- Support implementation of re-housing and stabilization plans
- Enforce shelter rules and program policies
- Perform other duties as assigned
Qualifications
- Bachelors degree in Social Workor related field preferred; equivalent experience accepted
- 25 years of experience in direct care, housing, or related services preferred
- Experience working with homeless or low-income populations preferred
- Valid drivers licenserequired
Skills & Knowledge
- Strong communicationand organizational skills
- Ability to manage multiple cases and priorities
- Knowledge of housing resources and social service systems
- Proficiencywith case management software and documentation
Work Schedule
- 3240 hoursper week; 8-hour shifts
- Weekday, evening, and weekendopenings
- Rotating on-call schedule
Physical Requirements
- Primarily in-person work
- Ability to sit or stand for extended periods
- Regular use of computer, phone, and office equipment
