**Exciting Opportunity for Home Health RN with OASIS Experience!**\n\nJoin our dynamic team as a Home Health RN and embark on a fulfilling career where you can truly make a difference in patients' lives! We are looking for compassionate professionals who thrive in a home health setting. \n\n**Key Responsibilities:**\n\n- Conduct home health visits to provide exceptional patient care\n- Utilize OASIS (Outcome and Assessment Information Set) for assessments\n- Coordinate and manage care plans in home health and hospice settings\n- Adapt to and manage changes in daily schedules efficiently\n- Travel to multiple patient locations while maintaining a positive attitude\n\n**Qualifications:**\n\n- Minimum of 2 years of experience in Home Health and Hospice\n- Proven OASIS experience is a must\n- Valid driver’s license in good standing\n- BLS certification required\n- Experience with EMR systems, specifically HealthWyse (Careficient), is a plus\n\n**Work Schedule:**\n\n- Monday - Friday, 8 am - 5 pm\n- Some weekend coverage is required\n- Occasional emergency call may be scheduled\n\n**Additional Information:**\n\n- Enjoy the autonomy of outpatient care while making impactful connections with patients\n- Background checks conducted by our facility\n\n**Why Catalytic Solutions (CatSol):** \nAt CatSol, we connect dedicated therapists with rewarding assignments across the country.
