Join Our Case Management Team
At HPI, our Case Managers play a vital role in guiding individuals and their families through the healthcare system. Rooted in the Case Management Society of America’s definition, our approach is a collaborative process focused on assessment, planning, facilitation, care coordination, evaluation and advocacy. Our goal is to meet the comprehensive health needs of our members while promoting
patient safety, quality of care, and cost-effective outcomes.
Our Case Managers —nurses, social workers and other healthcare professionals — serve as trusted partners helping members understand their conditions, access resources, and navigate care confidence.
What you’ll do:
As an HPI Case Manager, you’ll be dedicated to helping members achieve the best possible health outcomes. You will not only support individuals and their families but also contribute to the success of employer groups and the healthcare system as a whole.
In this role, you’ll also:
- Communicate with members/designees, physicians and other health care professionals regarding the Care Management process including information about the role of HPI and the Case Management program
- Telephonically assess members referred for Discharge Outreach services, for purposes of problem identification and referral to Case Management, when assigned to the Discharge Outreach Program, to ensure safe transition to next level of care upon discharge
- Determine and document quantifiable Case Management outcomes, including avoidable and actual dollar savings
- Advocate for options and services within the constraints of the health benefit plan to meet members’ health care needs
- Promote and facilitate excellent communication among the member/designee, family and health care team
- Continually educate and enable the patient/family in order to promote self-management and self-advocacy
- Research available community resources and alternative funding sources as appropriate
- Identify quality of care issues and escalate concerns to Management as appropriate
- Ensure documentation is objective and accurate
- Conduct utilization management of appropriate level of care and length of stay using standardized clinical criteria
- Consult with Physician Reviewers and other experts to determine appropriateness of care as needed
- Comply with all HIPAA standards for documentation, security and privacy
What you bring:
Minimum Education Requirements (must have documented proof*):
- Active, current and unrestricted professional licensure Registered Nurse (RN)or Licensed Social Worker (LCSW/LSW) in MA, ME, NH, or state of residency
Minimum Experience Requirements:
- Three or more years’ clinical experience as an Oncology RN, working in either a hospital or clinic setting
- Experience with utilization management utilizing standardized criteria such as InterQual or MCG
Preferred Education Requirements:
- Bachelor’s Degree in Nursing
- Certified Case Manager
Preferred Experience Requirements:
- Three or more years’ Discharge Planning and/or Case Management experience, Utilization Management experience, knowledge of Case Management Society of America Standards of Practice, and familiarity with URAC standards, Department of Labor, state/federal regulations
- Excellent customer service and communication skills, ability to prioritize and multi-task effectively, ability to work both independently and as a member of the team, recognize the need for follow-thru and follow-up, and ability to work within specified timeframes and meet deadlines
Computer Skills:
Proficient in Microsoft Office, including Word, Excel, Outlook and PowerPoint
