We are a nonprofit healthcare organization located approximately 30 miles north of Boston, dedicated to delivering a wide range of medical and supportive services to adults aged 55 and older through the PACE (Program of All-Inclusive Care for the Elderly) model. Our mission is to empower seniors to remain in their homes and communities safely for as long as possible by providing coordinated care that encompasses primary and specialty medical services, rehabilitation, social programs, meals, and transportation.
About the Position:

This role will primarily involve providing care within our skilled nursing facilities.

Work schedule is Monday to Friday, from 8 AM to 4 PM.

The average daily patient load ranges from 8 to 15 individuals.

Offer comprehensive and personalized care to enable seniors to live independently, safely, and comfortably in their own homes for as long as possible.

Engage in a collaborative care approach that includes physicians, nurses, occupational and physical therapists, social workers, behavioral health specialists, and certified nursing assistants (CNAs).
