We are looking for a compassionate Physical Therapist to join our team in Woburn, MA. We offer Full-Time, Part-Time, and PRN (Per Diem) opportunities to provide the best care to our residents. Sign-on bonus available for full-time employees!

Responsibilities:

• Provide physical therapy services to promote excellence in rehab care, interdisciplinary cooperation, and the quality of patient care.

• Evaluate and assess patients in a timely manner according to state, payor, or facility requirements.

• Develop individualized treatment plans and goals for patients, and provide treatment accordingly.

• Provide consultations and work collaboratively with other healthcare professionals to promote the best outcomes for patients.

• Communicate with staff, patients, families, and other disciplines as necessary for coordinated care.

• Document treatment plans, goals, and progress according to Medicare and other payor guidelines.

• Participate in family conferences and provide updates on patient progress as needed.

• Record and submit timely documentation and treatment charges per facility policy and procedure.

Qualifications:

• Current MA State License as a Physical Therapist.

• Ability to work as part of a dynamic and cooperative team, distributing the workload equitably and according to the department's needs.

• Strong ability to evaluate, assess, and plan individualized treatment plans for patients.

• Capability to educate, teach, and counsel patients, families, and staff on the rehabilitation process.
