GENERAL SUMMARY
Provides direct care and coordination of medical services to program participants.
ESSENTIAL JOB FUNCTIONS*
- Provides direct care, as needed.
- Provides training in relevant health topics for staff and clients, as needed.
- Coordinates medical services with each participant’s primary physician or medical clinic.
- Oversees all health care services provided to program participants.
- Participates in the ongoing interdisciplinary evaluation and review of each participant’s needs and status.
- Obtains reports and approval of treatment plans from primary physicians.
- Documents recommendation in participants’ records resulting from periodic reviews.
- Advises program management and staff of any medical problems that may hinder a participant’s progress or that may be negatively impacted by program activities.
- Completes, signs and/or reviews monthly progress notes in accordance with regulations established by the Commonwealth of Massachusetts-Executive Office of Health and Human Services (EOHHS).
- Serves as mandated reporter for suspected cases of abuse and/or neglect to the Disabled Persons Protection Commission (DPPC).

PREPARATION, KNOWLEDGE, SKILLS & ABILITIES
- Registered Nurse (RN) licensed by the Massachusetts Board of Registration in Nursing.
- Minimum 1 year prior experience in a community health setting.
- Must be able to effectively communicate in both verbal and written form with coworkers and supported individuals and read fluently.
- Valid driver’s license in the state of Massachusetts required.
- Successful completion of all state and Agency mandated trainings required upon hire.
- Must be compassionate and patient.
SUPERVISORY RESPONSIBILITY
- None.
WORKING CONDITIONS/PHYSICAL DEMANDS
- Frequent standing, walking, bending, and twisting.
- Ability to lift up to 50 pounds.
- Ability to pull up or transfer individuals weighing up to 250 pounds.
