Alliance Health at Baldwinville is a 94-bed long term care facility. We are looking for a PT to join our Rehab team in Skilled/Long Term Care and Outpatient Clinic.

Responsibilities include:

Evaluates and screens all patients referrals to the Rehabilitation Department for Physical Therapy.

Interprets evaluations and may consult with other rehab team members to assess goals and plan of treatment. Works in conjunction with the Interdisciplinary Team to develop the overall resident care plan. Work Remotely No

Education: Bachelor’s License/Certification: Physical Therapy License (Required)
