Position Overview: The Wellness Nurse (Licensed Practical Nurse) plays a crucial role in supporting the Resident Services Director in the effective delivery, coordination, and implementation of Resident Care Services within the community.
Key Responsibilities:
- Oversee the Resident Services Department staff, including Certified Nursing Assistants (C.N.A.s).
- Aid in managing the medication program and assist residents with self-medication.
- Foster positive relationships with residents, their families, and healthcare providers.
- Report any significant incidents or changes in residents' needs to the Executive Director or Resident Services Director.
- Assist in completing resident assessments and developing service plans.
- Support clinical assessments and recertifications in accordance with ALSA requirements.
- Collaborate with the Resident Services Director in hiring and managing departmental performance.
- Participate in the training and orientation of new staff members.
- Attend scheduled in-service training sessions and team meetings.
- Help with the scheduling and staffing of Resident Services Assistants as necessary.
- Provide coaching and mentorship to staff within the Resident Services Department.
- Support residents and their families to ensure a high level of satisfaction.
- Assist in supervising RSA staff during care provision.
- Perform additional duties as assigned.

- Adhere to emergency protocols.
- Understand and implement safety practices and procedures.
Communication:
- Effectively communicate with supervisors and colleagues.
- Maintain positive relationships with residents, families, peers, and other staff members.
- Promote a constructive work environment.
Education/Experience/Licensure/Certification:
- Current Licensed Practical Nurse license in the relevant state(s).
- Certification in basic first aid and CPR.
- Minimum of two years of full-time or equivalent clinical experience.
- Preferred experience in assisted living or long-term care settings.
- Interest or experience in working with seniors is a plus.
Physical Requirements:
- Must be in good physical and emotional health, free from communicable diseases.
- Ability to bend and reach as necessary.
- Capable of pushing, pulling, and lifting up to 50 lbs. when required.
- Able to stand for extended periods.
- Proficient in transferring residents and using a Hoyer Lift and other medical equipment as needed.
Miscellaneous:
- Availability to work weekends and holidays as assigned.
- May need to work shifts other than the one initially assigned.
- Potential for extended hours as required.
- May be exposed to infectious waste, diseases, and conditions.
- Could encounter residents who are hostile or emotionally distressed due to their mental state.
