Position Summary:
The Clinical Transition Coordinator is responsible for coordinating the facilitation of appropriate services for individuals with clinically complex needs working in conjunction with their teams during the transition from the school system into adult services and beyond. This person will review available information, observe individuals, recommend necessary assessments and evaluate needs as the transitioning individual prepares to move into the adult services system.
Essential Responsibilities:
· Plan, execute and coordinate the development of transitional plans with individuals eligible for services and deemed clinical upon intake
· Possess a working knowledge of Medicaid applications, guardianship petitions, Individualized Education Plan (IEP) development process and Mental Health Services
· Attend IEP and wrap-around meetings as necessary
· Demonstrate a high level of effective communication and documentation on client’s behalf
· Coordinate and develop Service Agreements per state regulations and agency policy and practice
· Monitor goals and objectives as established in Service Agreement
· Complete clinical referrals to appropriate service providers
· Demonstrate an understanding of expense and revenue for services
· Participate in individual budget decisions regarding funds distributed for external vendors and consolidated services
· Act as liaison, educator, and coach to families to identify and develop client centered community resources to meet individual needs
· Maintain knowledge of educational laws and guidelines, Division for Children, Youth and Families and Bureau of Elderly and Adult Services reporting
· Maintain professional appearance in community on behalf of our clients and the Agency
· Other duties and responsibilities as assigned by Manager
Requirements:
Qualifications:
Education and Experience
· Bachelor’s degree required, master’s degree in related field preferred
· Minimum of 3 years of related experience
· Experience working with individuals with intellectual and/or developmental disabilities, Acquired Brain Disorder and mental illness
· Proficiency in various software applications including Word, Excel, and Access
· Valid driver’s license and access to a safe operating vehicle during scheduled working hours
· $100,000/300,000 Automobile Liability Insurance
