Position Overview
We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join a Skilled Nursing and Rehabilitation Facility in Hadley, MA. This per-diem role involves delivering direct patient care, administering medications, and working collaboratively with interdisciplinary teams to support rehabilitation and recovery.
Key Responsibilities
- Conduct patient assessments and monitor vital signs
- Administer prescribed medications and treatments
- Provide wound care, catheter care, and other clinical procedures
- Collaborate with RNs, physicians, and therapists on care planning
- Maintain accurate and timely documentation of patient status and care
- Support rehabilitation goals through direct, compassionate patient care
- Comply with facility policies, safety protocols, and infection control standards

Requirements
Qualifications
- Education: Completion of an accredited LPN program
- Licensure/Certifications:
- Active Massachusetts LPN license
- Current BLS certification
- Experience: Minimum of 1 year in a skilled nursing, rehabilitation, or long-term care setting
- Technical Skills: Medication administration, wound care, EHR documentation
- Soft Skills: Effective communication, compassion, and adaptability in a fast-paced environment
