Responsibilities
- Provide direct patient care and carry out physician orders in compliance with NH nursing practice standards.
- Administer medications and treatments accurately and in accordance with facility policies.
- Assess, monitor, and document resident status; promptly report changes in condition to the RN or supervisor.
- Collaborate with the interdisciplinary care team to develop and update individualised care plans.
- Maintain professional, compassionate communication with residents, families, and colleagues.
Requirements
- Current, unencumbered LPN licence in New Hampshire — required.
- Minimum 1 year of experience in a long-term care or skilled nursing environment.
- Familiarity with MAR and TAR documentation systems preferred.
- Current BLS/CPR certification.
- COVID-19 vaccination or documented employer-issued exemption required.
