Retention bonus of $10,000 payable in quarterly installments during your first year of employment.

We are seeking a skilled and compassionate Home Care Physical Therapist to join our team and help our patients achieve their rehabilitation goals. This position is full time with hourly pay.
Responsibilities:
- Evaluate patients in their home environment to assess physical mobility, strength, and function.
- Develop and implement individualized treatment plans tailored to the needs of each patient.
- Provide hands-on physical therapy to improve mobility, reduce pain, and enhance daily living skills.
- Collaborate with interdisciplinary team members including nurses, occupational therapists, and physicians to ensure comprehensive patient care.
- Educate patients and their families on exercises, techniques, and self-care strategies for optimal recovery.
- Document progress and adjust treatment plans as necessary based on patient response and goals.
- Monitor and ensure patient safety during therapy sessions.

Requirements:
- Graduate of a program in physical therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association or its equivalent;
- One year of professional physical therapy experience under the appropriate supervision; or
- A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position.
- Possession of a current State of New Hampshire license to practice as a registered physical therapist.
- Must be able and willing to learn to utilize a portable electronic field device. Must be able to manage electronic patient health information (ePHI) remote access in accordance with HCS policies and procedures.
- Valid driver’s license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and auto liability insurance.
