Job description:
<ul>
<li>Setting: Skilled Nursing Facility, Sub-acute and long-term care patients</li>
<li>Schedule: Minimum of (2) days on site per week. Telehealth support is available on-site for virtual interactions</li>
<li>Must be licensed in VT</li>
<li>Experienced in SNF preferred, not required</li>
<li>Potential: 200-300K (Base Salary + RVU Bonus Model)</li>
