Park Avenue Healthcare Arlington, MA is short-term rehabilitation and long-term care center, seeking an Social Worker to join our team.
The right candidate must possess a Massachusetts license as an LICSW.
Duties and responsibilities include and but are not limited to:

psychosocial assessments

completion of the MDS- mood, behavior and cognition sections

quarterly assessments
-
passr, discharge planning
-
attend and provide input at care plan meetings

follow-up on complaints and grievances

provide ongoing support to residents and their families
