We are looking for an experienced RN MDS/CRC Coordinator who understands the connection between strong clinical documentation, thoughtful care planning, regulatory compliance, quality outcomes, and appropriate reimbursement. This is an ideal opportunity for an organized and collaborative nurse who enjoys analyzing details, leading interdisciplinary conversations, and helping teams turn resident information into meaningful plans of care.

What You’ll Do

  • Coordinate the facility’s Resident Assessment Instrument process, including required MDS, OBRA, and Care Area Assessments.

  • Maintain the assessment schedule and ensure submissions are completed accurately and within required regulatory timeframes.

  • Lead interdisciplinary care plan meetings and help ensure plans reflect each resident’s current needs, goals, and changes in condition.

  • Review and audit medical records to confirm that MDS entries are supported by complete and accurate documentation.

  • Educate and support team members when documentation or assessment opportunities are identified.

  • Serve as a resource regarding RAI requirements, regulatory guidance, and payment classifications under the Skilled Nursing Facility Prospective Payment System.

  • Collaborate with physicians, health plans, nursing staff, and other members of the interdisciplinary team regarding authorizations, eligibility requirements, and continuity of care.

  • Verify successful assessment submissions and promptly research and correct identified errors.

  • Participate in resource-utilization reviews and triple-check meetings before billing.

  • Assist with admissions, discharges, and resident care when needed.

  • Maintain current knowledge and competency through required education and training.

  • Communicate clearly with clinical staff, providers, health plans, and leadership.

  • Are analytical, organized, dependable, and comfortable working independently.

  • Are proficient with electronic medical records and Microsoft Office applications.

What You’ll Need

  • A current, active RN license in good standing.

  • At least three years of RN experience, including a minimum of one year in a supervisory role.

  • Previous MDS completion experience is preferred.

  • RAC training must be completed within six months of hire; RAC certification is preferred.

  • Availability to work additional hours, including occasional weekends or holidays, when necessary.
