**Exciting Opportunity for RN Case Managers in Home Health!**\n\n**Job Description:**\n\n- Join a passionate team where you truly get to know your patients and become an integral part of their care.\n- Experience broad independence while being supported by a cohesive patient-care team.\n- Enjoy a work/life balance that offers the flexibility you need.\n \n**Key Responsibilities:**\n\n- Develop comprehensive care plans in collaboration with physicians, patients, and families upon admission.\n- Periodically review and recertify patient care plans as appropriate.\n- Coordinate patient care through effective interdisciplinary communication and timely case conferences.\n- Assess patient clinical, functional, and emotional status to identify problems and track progress.\n- Document evaluations, treatment goals, and objectives with precision.\n- Teach patients and caregivers self-care techniques and provide necessary instructions as directed by the physician.\n\n**Minimum Qualifications:**\n\n- Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license.\n- Graduate of an accredited school of nursing.\n- Minimum of 2 years of experience in Home Health.\n\n**Shift Information:**\n\n- Full Time, Days\n\n**Why Catalytic Solutions (CatSol):**\n\nAt CatSol, we connect dedicated therapists with rewarding assignments across the country.
