About the Role:

The Visiting Nurse - Registered Nurse (RN) plays a critical role in delivering high-quality, compassionate healthcare services directly to patients in their homes. This position focuses on assessing patient health status, developing and implementing individualized care plans, and providing skilled nursing care to promote recovery, manage chronic conditions, and improve overall well-being. The Visiting Nurse collaborates closely with physicians, therapists, social workers, and family members to ensure coordinated and comprehensive care. This role requires adaptability to diverse patient needs and environments, ensuring safety and adherence to clinical protocols. Ultimately, the Visiting Nurse serves as a vital link between the healthcare system and patients, enhancing access to care and supporting patient independence in the community setting.

Minimum Qualifications:

  • Current and valid Registered Nurse (RN) license in the state of practice within the United States.
  • Graduation from an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  • Minimum of one year of clinical nursing experience, preferably in home health, community health, or related settings.
  • Basic Life Support (BLS) certification from the American Heart Association or equivalent.
  • Ability to pass background checks and meet health and immunization requirements.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) degree.
  • Home Health Nursing certification (e.g., Certified Home Health Nurse - CHHN).
  • Experience with electronic health records (EHR) and telehealth technologies.
  • Strong knowledge of chronic disease management and palliative care.
  • Excellent communication and cultural competency skills to serve diverse patient populations.

Responsibilities:

  • Conduct comprehensive patient assessments to evaluate physical, emotional, and social health needs in the home setting.
  • Develop, implement, and regularly update individualized nursing care plans based on patient assessments and physician orders.
  • Administer medications, treatments, and therapies as prescribed, while monitoring patient responses and documenting outcomes accurately.
  • Educate patients and their families on disease management, medication adherence, wound care, and health promotion strategies.
  • Coordinate with interdisciplinary healthcare teams to facilitate seamless communication and continuity of care.
  • Maintain accurate and timely clinical documentation in compliance with regulatory standards and organizational policies.
  • Identify changes in patient conditions promptly and communicate concerns to the appropriate healthcare providers.
  • Ensure adherence to infection control protocols and safety standards during all patient interactions.
  • Provide emotional support and counseling to patients and families to help them cope with illness and recovery processes.
  • Manage scheduling and travel efficiently to maximize patient visit effectiveness and coverage.
