Position Overview

CareOne is seeking a dedicated Physical Therapist (PT) to join our in-house rehabilitation team. Unlike many skilled nursing organizations that rely on external contractors, CareOne provides rehabilitation services internally, offering our therapists a consistent, collaborative, and career-advancing work environment. The PT is responsible for providing care to residents at the highest level, ensuring optimal functional outcomes and quality of life.

Key Responsibilities

Clinical Care & Resident Services:

  • Provide and direct rehabilitation care in accordance with accepted professional practices, meeting all center, state, federal, and accrediting standards.
  • Evaluate resident needs and develop individualized, effective, and efficient treatment plans.
  • Manage and coordinate treatment plans with other staff and disciplines as necessary.
  • Participate in resident screenings according to established schedules and practices.
  • Provide feedback to the designated supervisor on clinical and rehabilitation issues.

Documentation & Compliance:

  • Maintain complete, accurate, and timely clinical documentation to meet center, state, federal, and accrediting agency requirements.
  • Ensure billing records are accurate and compliant with regulatory guidelines.
  • Participate in center-directed processes, services, and quality improvement initiatives.

Professional Development & Collaboration:

  • Participate in in-service training programs and continuing education opportunities.
  • Collaborate daily with nursing and clinical staff to ensure optimal patient outcomes.
  • May travel to other CareOne facilities to provide services as needed.

Qualifications

Required:

  • Master’s or Doctorate degree in Physical Therapy.
  • Current and active Physical Therapist license in the state of practice.

Preferred:

  • Experience or knowledge in older adult rehabilitation.
  • Familiarity with governmental and regulatory guidelines in designated healthcare settings.
