Responsibilities:

• Assist in overseeing the development, implementation, supervision, and evaluation of the Social Services Department
• Complete comprehensive assessments for assigned residents and develop individualized care plans
• Identify and promote individualized, non-pharmacological approaches to care that meet the mental and psychosocial needs of each resident
• Ensure residents have access to appropriate treatment and resources
• Treat all residents with dignity and respect, promoting and protecting their rights
• Establish a culture of compliance by adhering to all facility policies and procedures
• Maintain confidentiality of protected health information and adhere to all facility policies and procedures
• Perform administrative requirements, including completing assigned reports and submitting them to the Social Services Director
• Contribute to the facility's efforts to maintain and improve quality of care through participation in quality improvement initiatives

Requirements and Qualifications:

• LSW - Licensed Social Worker.
• One year of supervised social work experience in a healthcare setting
• Ability to read, write, speak, and understand the English language
• Ability to make independent decisions and deal tactfully with personnel, residents, family members, visitors, and government agencies
• Must be supportive of the team, contribute to, and be an example of teamwork
• Basic computer skills
• Ability to move intermittently throughout the workday and perform other common physical movements as needed for the position

Working Conditions:

• May be subject to falls, burns from equipment, and/or odors throughout the day
• Encounter reactions from dust, tobacco smoke, disinfectants, and other air contaminants
• Subject to exposure to infectious waste, diseases, and/or conditions
• May be subject to hostile or emotional residents, family members, visitors, or personnel
• Ability to work beyond normal working hours, on weekends, and holidays when necessary
