HCS is seeking an experienced Physical Therapist (PT) to make home visits. The Physical Therapist provides and directs the provision of home physical therapy, based on agency policy and procedures, through the competent application of the therapy process.


Retention Bonus of $6,000 payable in quarterly installments during your first year of employment.

Requirements:

  • Graduate of a program in physical therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association or its equivalent;
  • One year of professional physical therapy experience under the appropriate supervision; or
  • A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position.
  • Possession of a current State of New Hampshire license to practice as a registered physical therapist.
  • Must be able and willing to learn to utilize a portable electronic field device. Must be able to manage electronic patient health information (ePHI) remote access in accordance with HCS policies and procedures.
  • Valid driver’s license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and auto liability insurance.
