Responsibilities

Manage patient care plans and coordinate services for optimal outcomes.

Conduct assessments to identify patient needs and develop care strategies.

Collaborate with healthcare teams to ensure comprehensive patient support.

Experience/Qualifications

Current RN license in Vermont with case management experience.

Proven ability to develop and implement effective patient care plans.
