We're seeking a highly skilled Director of Case Management to oversee case management operations in a leading acute care hospital located in beautiful Manchester, NH. In this executive role, you'll drive the strategic direction of discharge planning and care coordination across interdisciplinary teams while ensuring regulatory compliance and resource efficiency.

Key Responsibilities

  • Provide strategic and operational leadership for the Case Management Department.

  • Oversee all aspects of hospital discharge planning, clinical resource management, and care transitions.

  • Ensure compliance with organizational policies and regulatory standards (e.g., CMS, Joint Commission).

  • Collaborate with internal departments and external partners to optimize care coordination and continuity.

  • Lead and develop a team of case managers, social workers, and support staff.

  • Analyze data to track department outcomes, drive quality improvement, and manage performance metrics.

  • Serve as a key liaison between clinical teams, administration, and community resources.

  • Manage daily operations, budget planning, and staffing within the department.

Qualifications

  • Bachelor's degree in Nursing or related healthcare field (required)

  • Master's degree in Nursing, Health Administration, or Business Administration (preferred)

  • Current licensure as an RN, LCSW, or LMSW (required)

  • Minimum 3+ years of hospital case management experience in an acute care setting (required)

  • Minimum 2+ years of case management leadership experience (required)

  • Strong communication, team leadership, and presentation skills

  • Demonstrated success in achieving organizational goals and improving care outcomes
