As a compassionate and dedicated Social Worker, you will join our collaborative care team at Carease Health - MA, providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. Our mission is to prioritize proactive measures for enhanced physical health and well-being, minimizing hospitalizations and elevating patient care standards.

Salary: $28,000 - $30,000 ANNUALLY

Job Summary:

As a Social Worker, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. This role is ideal for professionals who want to make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment.

Key Responsibilities:

  • Visit designated facilities in accordance with the schedule set with your supervisor

  • Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs

  • Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns

  • Document visit notes and maintain detailed records of interactions and interventions after each visit

  • Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications:

  • Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health-related field

  • Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

  • Strong communication, documentation, and relationship-building skills

  • Education: Associate Degree

  • Experience: 1 year
