Job Details
• Provide occupational therapy services to patients in their place of residence who have impairments, cognitive and/or physical resulting from injury, disease, disability or other causes.
• Patients may range in age from birth to over 100 years.
• Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs.
• Manage technical, environmental and financial resources.

Job Requirements
• 1 year certified home care experience.
• Occupational Therapist Licensure.
• Two years of experience in acute care, rehab or homecare as an Occupational Therapist.
• Registered with the Board of Registration in Massachusetts.
• Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred.
• Strong customer service skills and ability to work with diverse populations.
• Excellent verbal, written and electronic communications skills.
• Ability to prioritize work and adapt to changing situations.
• Travel required within PHH's geographic area.

Schedule Information
• Contract length is 13 weeks.

Additional Details
• MGB Badge Photo required for onboarding: color photo taken in past 6 months, plain background, no filters or eyeglasses.
• MGB MVR Check required within 45 days of start for all home care positions.
• License/Certificate Verification required within 30 days of start date and updated upon expiration; MA Conditional Licenses not accepted.
• N95 Fit Test required within 365 days to start date and updated annually; no religious exemptions.
• MMR vaccination or proof of immunity required.
• Influenza Vaccine required during flu season (Sept 15 - April 30); no exemptions.
• TB Test documentation required within 3 months of start date and updated if >6 month break.
• Health Statement signed by MD/DO/NP/PA required within 365 days of start date and updated if >6 month break.
• MGB Criminal Background Check required within 45 days prior to start date and updated if >6 month break.
• MGB Massachusetts CORI required within 30 days prior to start date and updated if >6 month break.
• MGB Resume including past 7 years of work experience required for submission.
• MGB Skills Checklist required for submission.
• MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions.
• MGB References: 2 recent references, minimum 1 Supervisor or Above, in Skillset.
• Respirator Medical Clearance Attestation required if fit test is required.
• MGB Education Verification through background check company.
• MGB SSN Trace required with Criminal check.
• MGB National Sex Offender Check required within 30 days of start and updated annually.
• MGB Exclusion Checks required within 30 days of start and annually.
