Location: Amherst, NH

Position: Part-Time to Full-Time Growth Opportunity

Compensation: $55 - $68 per hour, guaranteed 20 hours per week to start, with incentives and exponential room for growth

Connective Wellness is seeking a highly skilled, forward-thinking practitioner—Medical Doctor, Doctor of Osteopathic Medicine, or Nurse Practitioner—with expertise (or deep interest) in functional medicine, women’s health, complex chronic conditions, and concierge-style primary care.

Who We Are


Connective Wellness is a patient-centered, integrative healthcare practice specializing in complex chronic conditions such as Ehlers-Danlos Syndrome (EDS), POTS, MCAS, chronic pain, autoimmune disorders, and hormonal imbalances. We take a multidisciplinary approach, combining functional medicine, primary care, physical therapy, nutrition, IV hydration, and patient advocacy to provide comprehensive, root-cause care that truly supports our patients' needs.


Our practice is built on a direct-pay, concierge-inspired model that allows providers to spend more time with patients, focus on whole-person health, and offer personalized treatment plans—without the restrictions of traditional insurance-based care. We are rapidly growing and looking for like-minded providers who are passionate about transforming healthcare, challenging outdated systems, and delivering exceptional care to an underserved population.


Who We’re Looking For


  • A licensed practitioner (MD, DO, FNP, or AGNP) with a strong background in functional, integrative, or precision medicine.
  • Someone ready to act as a true primary care provider in a self-pay, concierge model.
  • Passionate about complex, multisystemic conditions like EDS, POTS, MCAS, and women’s health (or eager to train in these specialties).
  • A strong background in women’s health and hormonal care is strongly preferred.
  • Comfortable with medical pain management, including prescribing LDN when appropriate.
  • Excited to oversee and expand our IV hydration program for patients with dysautonomia, chronic illness, and GI issues.
  • A true team player—flexible, adaptable, and eager to build something new.
  • Comfortable with a varied schedule across different days and weeks as we scale this specialty.
  • Someone who enjoys the flexibility of part-time hours initially but with full-time availability in the near future, knowing that this role will grow to full-time as we build a steady patient load.
  • Likely a great fit for someone already working per diem elsewhere who’s looking for the right long-term opportunity.
  • Would prefer a primary care background but one that leans toward functional medicine and root-cause approaches.

  • Committed to in-clinic patient care—while this role may include telehealth, those virtual appointments will be conducted from the clinic.


Applicants must have an active New Hampshire medical license (MD, DO, or NP) or be eligible to obtain one, be willing to get licensed in additional states for telehealth, and carry their own malpractice insurance (Connective Wellness will also provide coverage under our clinic policy). Additionally, they must have an active DEA license or be able to obtain one as part of their role.


Your Role + Responsibilities


  • Primary care for medically complex patients: Our patients are highly complex and have often faced years or decades of medical gaslighting. As their provider, you will offer comprehensive, patient-centered care that includes:
  • Annual exams and preventative care
  • Complex care follow-ups for multisystemic conditions
  • Medical pain management, including LDN and integrative approaches
  • Acute care visits for infections, injuries, or symptom flare-ups
  • Medication management and prescribing when appropriate
  • Hormonal health and functional medicine interventions
  • Care coordination with specialists outside Connective Wellness when needed
  • Building meaningful, long-term patient relationships: As a part of our concierge medicine model, you will have more patient contact than in a traditional primary care setting. This means fostering trust, validating patient experiences, and becoming a true partner in their healthcare journey. You will engage with patients between visits through our OhMD texting platform and Jane EHR system, ensuring they feel supported and heard every step of the way. You’ll also collaborate seamlessly with our multidisciplinary team to provide integrative, team-based care that makes a real difference.
  • Comprehensive diagnostics + treatment planning: Work within a multidisciplinary team using the most advanced, multisystemic diagnostic process in the field for EDS, integrating the Prince-Cole Scale™, validated assessments, and a whole-body systems approach to uncover the full picture of a patient’s health.
  • Women’s health + hormonal care: Many of our patients experience hormonal imbalances, menstrual irregularities, perimenopause/menopause concerns, and complex endocrine issues related to EDS, POTS, and MCAS. You will provide comprehensive assessments, hormone testing, and individualized treatment plans using both functional and conventional medicine approaches. This may include bioidentical hormone therapy (if within scope), thyroid management, adrenal health support, and cycle tracking for symptom management. You will work closely with patients to identify root causes, optimize hormonal balance, and improve overall quality of life.
  • Medical pain management: Includes prescribing Low-Dose Naltrexone (LDN) and working with a team-based pain management approach.
  • Oversight of IV hydration program: Managing protocols for patients needing hydration and nutrient therapy.

Benefits + Incentives


At Connective Wellness, we are committed to investing in our team’s success and well-being. As a growing practice, we offer competitive benefits and incentives, with plans to expand offerings as we scale.


  • Generous PTO: 15 days PTO per year (at part time; 30 days per year at full time), plus all federal holidays paid, and paid time off between December 24 and January 2.
  • $200 monthly healthcare stipend: To support your health and wellness while we work toward offering full health insurance by December 2025.
  • $250 patient referral bonus: For every patient you refer who completes their 4th appointment at the clinic.
  • $1,000 clinician referral bonus: When you refer a provider who successfully completed their first 3 months with us.
  • Direct patient growth incentive: Earn 25% of the first visit fee for every new patient you personally bring into the clinic (whether through personal outreach, community connections, or existing relationships).
  • Concierge subscription enrollment bonus: Earn $150 per patient who signs up for our concierge medicine subscription model based on your care and recommendation.
  • Flexible growth potential: Opportunity to significantly expand your role, increase hours, and grow into a leadership position as we scale our concierge medicine model. We expect this role to become full-time within 4-6 months of hire.
