We are expanding to 11 more towns covering the south shore area, including Quincy, Braintree, Weymouth, etc.OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, safe living for our consumers, we hope to allow them to stay active and engaged in their lives and in their own communities for as long as possible.Our mission: Through the talents of an experienced and diverse workforce, OCES supports the independence and dignity of older adults and individuals with disabilities by providing essential information and services that promote healthy, safe living which positively impacts our community.We are seeking a Case Assistant to provide administrative support for our Community Transition Liaison Program (CTLP).Job Duties/Responsibilities:• Supports the Community Transition Liaison to operate at the top of their role and responsibility • Accompanies the Liaison on nursing facility visits as needed • Provides additional support to the Liaison for complex cases • Gathers documentation needed to assist the consumer in applying for public benefits • Assists the consumer in completing and submitting housing applications • Conducts clerical duties as needed by Liaison, including data entry into EOEA-designated cloudbased data enterprise system • Ensures case documentation meets the standards set forth by the Executive Office of Elder Affairs for the Community Transition Liaison Program • Providing accurate and reports as required by the department • Assist staff in tracking waiver and recipient choice forms as needed • Other duties as assigned.What you will bring:High School diploma or equivalent, Associates preferred.One-year relevant experience.Excellent written and verbal skills.Excellent computer skills.Ability to work with statistical and financial data.Knowledge of general office procedures and equipment. Proficient with Microsoft Office Suite or related software.
