Eminence Home Care is looking for a compassionate and organized Home Care Coordinator to join our team. In this role, you will be responsible for coordinating care services for our clients, managing caregiver schedules, and ensuring seamless communication between clients, caregivers, and healthcare professionals.
You will play a vital role in maintaining the quality of care and client satisfaction by effectively managing daily operations and addressing any concerns promptly.
Responsibilities:
- Coordinate and schedule home care services to meet client needs.
- Communicate regularly with clients, caregivers, and family members to ensure satisfaction and address issues.
- Manage caregiver assignments and schedules efficiently.
- Maintain accurate records and documentation related to client care and scheduling.
- Assist in recruitment and training of caregivers as needed.
- Ensure compliance with company policies and regulatory requirements.
Location: Manchester, NH
Hours: Monday to Friday, 8:30am - 5:00pm
Requirements
- Previous experience in home care coordination or related healthcare administrative roles preferred.
- Excellent organizational and communication skills.
- Ability to multitask and handle urgent requests effectively.
- Proficient in using scheduling and home care management software.
- Strong problem-solving skills and attention to detail.
- Ability to pass background checks and any required screenings.
- Fluency in English, both written and verbal.
