Our organization is excited to announce an opportunity in the rapidly growing field of Sub-Acute and Long-Term Care, which offers excellent work-life balance with no on-call duties or weekend shifts! We are looking for a Full-Time provider (32-40 hours weekly) to fill a float position between two facilities located in Keene, NH. This role is open to both nurse practitioners and physician assistants, and we encourage new graduates to apply.
The annual base salary for this position ranges from $112,320 to $124,800, along with participation in our incentive compensation bonus program, which has no cap on earnings.

Position Responsibilities
The Advanced Practice Provider will report directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. This role involves collaboration with the Center Leadership Team to meet specific goals and priorities, as well as working alongside the Medical Director and/or Attending Physician to deliver patient-centered medical care.
Key responsibilities include:
- Providing primary and urgent care services to residents in nursing centers.
- Collaborating with other medical providers to ensure high-quality healthcare delivery.
- Working with Center Leadership to fulfill our mission of improving lives through compassionate care.
Qualifications
To qualify for this role, candidates must meet the following criteria:
- Graduation from an accredited School of Nursing program and licensure as a registered nurse.
- Certification as a Nurse Practitioner from an accredited Nurse Practitioner Program with a Master's Degree in Nursing, specializing in Older Adult and/or Family Nurse Practitioner (FNP).
- For Physician Assistants, graduation from an ARC-PA accredited program and successful completion of the PA National Certifying Exam (PANCE) to obtain the PAC designation.
- A minimum of two years of experience working with adult or geriatric populations is preferred.
Working Conditions and Support
Advanced Practice Providers must hold an unrestricted professional license and DEA for each state in which they practice. We offer a comprehensive 90-day training program along with ongoing support.
