Job Description

Epic Nursing Services is seeking a full time Program Case Manager to assess the needs, initiate and coordinate services for frail elders and disabled adults enrolled in agency programs. The Case Manager will manage a caseload of approximately 150 clients to conducts in-home visits every other month, participates in an inter-disciplinary team and works collaboratively with community agencies.

Qualifications.

  1. A bachelor’s or master's degree in social work, human services or a related field required.
  2. One-year elder care and disabled adults case management experience preferred.
  3. Excellent computer skills required, including thorough knowledge of Microsoft Word, Outlook and Excel. Precise record keeping, excellent oral and written communication skills and a commitment to teamwork is essential.
  4. Excellent assessment and documentation skills required.
  5. Ability to work independently and be self-motivated.
  6. Spanish and Portuguese speaking candidates are encouraged to apply.
  7. Must have MA driver's license the agency will provide transportation to conduct home visits.

Responsibilities.

  1. Assist elderly and disabled consumers in developing and maintaining maximum independent living within the community
  2. Responsible for assessing the needs of program participants and to develop, monitor and review service plans for all clients
  3. Position requires home visits to elders to coordinate service plans, monitor Members progress, and corresponding paperwork, data entry and statistics.
  4. Obtain a social history and conduct a psychosocial evaluation.
  5. Complete a case-management progress note corresponding with each on-site visit or encounter and upon significant change.
  6. Review care aide logs daily or at a minimum weekly
  7. Review and document the fire and safety procedures for the qualified setting, participating in discharge and transition planning and implementation.
  8. Evaluate, support, and train care aides.
  9. Provide timely assistance and respond to urgent or emergency needs of the member
  10. Assists in developing an outreach plan, identifying priorities and a reasonable timetable
