Strong communication skills, both written and verbal, a dedication to guiding patients towards health-promoting behaviors, and the ability to comprehend and integrate complex clinical information are essential. The capacity to work independently as well as collaboratively is crucial. Key Responsibilities
- Medication Administration
- Adheres to organizational policies and procedures for the administration and documentation of medications.
- Completes the medication reconciliation process in accordance with established protocols.
- Follows safe procedures for administering medications and vaccines.
- Care Planning and Documentation
- Creates, discusses, and communicates a prioritized care plan for each patient.
- Develops, assesses, and updates individualized care plans, documenting outcomes accordingly.
- Initiates patient outreach, identifies needs, and implements appropriate interventions while evaluating their effectiveness.
- Continuously assesses and reassesses care needs within established nursing practices.
- Documents all patient care in compliance with nursing policies, standards, and relevant disease processes.
- Evaluates and documents patient responses to both pharmacological and non-pharmacological interventions.
- Advocates for patients and families, seeking assistance when they cannot represent themselves.
- Addresses ethical concerns with the clinical team and seeks resources for ethical decision-making.
- Collaborates effectively with healthcare team members across various specialties.
- Facilitates care transitions to ensure patient needs are met in appropriate settings and timely manners.
- Safety and Quality
- Confirms patient identity using two identifiers before any procedure, including medication administration and care delivery.
- Adheres to CDC hand hygiene guidelines and universal precautions, utilizing personal protective equipment as necessary.
- Ensures the work environment meets all regulatory standards.
- Understands individual roles in emergency preparedness and ambulatory codes.
- Professional Development
- Takes responsibility for personal professional growth by integrating evidence-based practices and performance improvement initiatives.
- Completes a minimum of 5 contact hours annually in relevant practice areas.
- Operates within the legal framework of the nursing practice act and supervises assigned personnel.

Job Requirements
- Education: Graduate from an accredited nursing program; BSN preferred.
- Experience: Previous experience in ambulatory care nursing is preferred.
- Licenses: Must hold a valid RN license.
- Certifications: Basic Life Support (BLS) certification required.
- Skills: Proficiency in ambulatory care nursing, strong communication skills, adaptability to changing priorities, and basic computer skills are essential.
- Demonstrated abilities in communication, interdisciplinary collaboration, problem-solving, and critical thinking are necessary.
- Familiarity with health system and community resources is preferred.
