There is a 40 hour/week or a 30 hour/week schedule available. The Nurse Care Coordinator will ensure patients have access to care and services through the Health Care for the Homeless (HCH) program. This role will focus on addressing the complex medical and behavioral health needs of the uninsured, underinsured, those experiencing homelessness and housing insecure individuals by utilizing the nursing process to conduct comprehensive assessments and identify co-occurring medical, mental health, and substance use disorders. Additionally, the Nurse Care Coordinator will provide care for patients with substance use disorders participating in the Medication Assisted Treatment (MAT) Program. Responsibilities include care coordination, chronic disease management, counseling, and health education. 
 
Responsibilities;
 
· Perform triage and rapid clinical assessments for patients who present at HCH shelter-based clinic and non-shelter-based clinic, in partnership with the HCH behavioral health team.
· Initiate appropriate emergency response in life-threatening or unusual situations to stabilize patients; promptly and directly notify the HCH provider(s).
· Assess and document patients’ vital signs, physical symptoms and chief complaints in preparation for their clinic visit.
· Perform comprehensive bio-psycho-social assessments and conduct mental health and substance use disorder (SUD) screenings using evidenced based tools.
· In collaboration with the interdisciplinary team, develop individualized care plans based upon nursing assessment as indicated. Provide health education and counseling to patients in support of self – care and health management goals.
· Based on the assessment, refer patients to the appropriate level and category of care within the multidisciplinary HCH team, and effectively communicate assessment findings and screening results to medical and behavioral health providers.
· Ensure all patients’ clinical assessments, diagnostic results and consult reports are up to date in the electronic medical record (EMR) in preparation for clinic visits with medical and behavioral health providers.
· Coordinate all aspects of care to ensure continuity of care within the HCH team and to ensure access to all required diagnostics, specialty care and services in the community.
· Coordinate discharge plans for each clinic visit; arrange referrals for diagnostic and specialty care, with support for transportation, escort, translation and medication assistance as needed.
· Perform nursing care interventions as ordered by the providers, and in support of the providers, for medication administration, point of care testing, and diagnostic procedures during each clinic session.
· As scheduling permits, participate in the Street Medicine program in collaboration with the HCH staff to identify, engage, and enroll individuals experiencing homelessness into HCH care.
 
MAT-Specific Patient Assessment &amp; Care Coordination:
 
· In collaboration with the Behavioral Health Provider, perform initial assessments including medical and mental health, medication, substance use and social history.
· Communicate and meet with the MAT treatment team and other HCH team members on a regular basis, employing a harm reduction approach to patient care.
· Provide education to patients and the community about access to MAT treatment programs.
· Assess and closely monitor patients through the induction, stabilization and maintenance phases of treatment.
· Manage scheduled nursing visits, urine toxicology screens, initial and ongoing lab testing, patient education and medication refills, as ordered by protocol or medical provider.
· In collaboration with licensed prescribing physicians, APRNs, PAs, and pharmacists, follow State and Federal guidelines in providing care to patients with substance use disorders.
Work with the prescriber to obtain prior authorizations as needed

TITLE: Producing Branch Manager
REPORTS TO: VP Regional Manager
SUPERVISES: Branch Staff
FLSA CLASS: Exempt

BASIC FUNCTION:
The Branch Manager (BM) is responsible for the managing of his branch staff, profitability of the branch as well as originating their own sales by contacting prospective clients and by developing and maintaining referral sources. The BM has flexibility in setting their working hours as well as scheduling the tasks performed in a work day. The BM is compensated on a commission only basis. 
ESSENTIAL DUTIES:

<li>Principal duty is the managing the profitability of the branch be ensuring the branch is properly staffed. </li>
<li>Management of staff in branch in regards to discipline, training, coaching. </li>
<li>Origination of mortgage loans.</li>
<li>Compliance with all state &amp; federal laws. </li>
<li>Generates referral business, leads and relationships with individuals.</li>
<li>Conducts sales calls, attends realtor sales meeting, functions and trainings.</li>
<li>Takes thorough and complete loan applications; analyzes and prequalifies applicant information to determine viability of the client’s desired loan program.</li>
<li>Pulls credit, analyzes credit, and compares for accuracy with the mortgage borrower’s statements</li>
<li>Corrects inaccuracies and determines correct loan options for the borrower based upon the applicant’s present situation, future plans and preferred loan product.</li>
<li>Provides guidance and information relevant to the risks and benefits of each program the applicant is qualified for.</li>
<li>Effectively explains and communicates the terms of each product and ensures all documents necessary are signed by the borrower.</li>
<li>Gathers all initial documentation required based upon the applicant’s selected loan program including disclosure requirements.</li>
<li>Provides loan package to the loan processor for review, stacking and pre-submission audit.</li>
<li>Works with the loan processor to provide any letters of explanation, or additional documentation as necessary and required by underwriting to clear all conditions and close each loan.</li>
<li>Maintains a working file for the office as required for compliance.</li>
<li>Acts as a liaison for all parties to the transaction, i.e. buyer, seller, realtor, builder, title company, appraiser, credit vendors, etc.</li>

Requirements:
KNOWLEDGE, SKILLS AND ABILITIES:
<ul>
<li>Leadership skills</li>
<li>Thorough understanding of company hiring policies and procedures.</li>
<li>Thorough understanding of mortgage products, pricing and guidelines.</li>
<li>Up to date understanding of all regulatory compliance and participate in various required trainings HarborOne Mortgage provides.</li>
<li>Ability to take accurate and thorough loan applications</li>
<li>Possess a working knowledge of credit reports, DTI, LTV, and CLTV ratios</li>
<li>Have excellent communication and interpersonal skills</li>
<li>Attention to detail and outstanding customer service is critical to success</li>
<li>Ability to create, implement and adapt a market penetration strategy for lead development, prospect to client conversion, and client to close conversion.</li>
<li>Ability to self source business through sales activities including cold calling, appointment setting, and group presentations.</li>
<li>Ability to create marketing systems to source new business as well as keep in touch with existing clients, realtors, builders, etc.</li>
<li>Experience with Google documents and other similar software preferred.</li>
</ul>
EDUCATION:
<ul>
<li>NMLS Licensed Loan Originators that meet state licensing requirements in each state they plan to do business in. </li>
<li>A minimum 5 years of Mortgage Origination experience with a proven track record is required.</li>
<li>Prior supervisory experience </li>
</ul>
PHYSICAL DEMANDS:
While performing the responsibilities of this position, the employee is required to effectively communicate verbally. The employee must be able to effectively operate a keyboard. The employee is occasionally required to stand, walk, and reach with arms and hands and occasionally lift and/or move up to 10-15 pounds. Vision abilities is required by this job include close vision.
