Milieu Clinicians use formal counseling and therapy techniques in otherwise less conventional treatment settings. BAYADA Home Health Care is currently seeking an experienced Physical Therapist Assistant (PTA) for a full time opportunity to perform home health visits throughout Franklin County, VT. This position will focus on providing services for adult and geriatric clients on a per visit basis.One year prior clinical experience as a licensed Physical Therapist Assistant is required. 
As a home care Physical Therapist Assistant (PTA), you will assist clients in attaining or maintaining optimal physical well being by providing treatment, tests, client education and appropriate interventions as delegated by a licensed Physical Therapist.

Each Physical Therapist Assistant (PTA) must:

<li>Have two years of appropriate experience as an Physical Therapist Assistant (PTA) and a satisfactory grade on a proficiency exam conducted, approved or sponsored by the U.S. Public Health Service when initial qualification occurred before December 31, 1977</li>
<li>Have at least one year’s work experience under the supervision of a qualified Physical Therapist.</li>

<li>Make home visits to clients in designated geographic territories</li>
<li>Perform assigned duties, including administration of medication, wound care, treatments, and procedures</li>
<li>Monitor clients' conditions; reporting changes to Clinical or Client Services Manager</li>
<li>Follow up with, execute, and properly document doctors' orders</li>
<li>Perform client assessments as necessary</li>
<li>Case management and coordination</li>
<li>Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet</li>
</ul>
Qualifications for a Registered Nurse (RN):
<ul>
<li>A current license as a Registered Nurse in Vermont</li>
<li>Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.</li>
<li>Ability to work independently and manage time effectively.</li>
<li>Strong interpersonal skills.</li>
<li>Solid computer skills; prior experience with electronic medical records (EMR) preferred.</li>
<li>Ability to travel to cases as assigned. </li>
