Job DescriptionLicensed Practical Nurse (LPN) – Home Health | Pittsfield, MAWe are seeking an experienced Home Health LPN for a contract opportunity in Pittsfield, MA.
Schedule & Responsibilities:
- Provide skilled nursing care to patients in their homes.
- Assess, implement, and document individualized patient care plans.
- Collaborate with physicians, caregivers, and the interdisciplinary healthcare team.
- Educate patients and families on health management and treatment plans.
- Maintain accurate clinical documentation.
- Participate in an on-call rotation as needed.
- Travel between patient homes using your own reliable vehicle.
Requirements:
- Active Massachusetts LPN License (must be in hand at submission).
- Active BLS Certification.
- Minimum 1 year of acute care nursing experience (hospital experience preferred).
- Home Health experience required.
- OASIS proficiency strongly preferred.
- Infusion therapy experience preferred.
- Valid driver's license, auto insurance, and reliable personal vehicle.
- Strong communication, critical thinking, and independent clinical decision-making skills.
Additional Information:
- Candidates must reside more than 50 miles from the facility.
- Requested time off must be disclosed at the time of submission.
- Orientation provided upon assignment start.
