Responsibilities:
- Provide direct patient care including bathing, grooming, dressing, and personal hygiene assistance
- Monitor and record vital signs, reporting changes in patient condition to nursing staff
- Assist patients with mobility, transfers, and ambulation using proper body mechanics and safety protocols
- Support patients with meals, hydration, and nutritional needs
- Maintain a clean, safe, and comfortable environment for patients and residents
- Document care activities accurately in accordance with facility policies and compliance standards
- Communicate effectively with patients, families, and interdisciplinary care teams
Requirements:
- Active Vermont Licensed Nursing Assistant (LNA) certification in good standing
