Case Management Nurse (RN) – Full Time
Location: Springfield, MA
Pay: Starting at $36.06 per hour
Schedule: Monday-Friday 9:00am-5:00pm with flexibility for occasional nights and weekends
$500 Sign-On Bonus! ($250 after successfully completing your first week of training, and $250 after successfully completing your first 90 days of employment)

Working under limited supervision, you'll support Adult Foster Care members by conducting clinical assessments, coordinating care, completing prior authorizations, and ensuring quality program services. Responsibilities
- Conduct clinical eligibility assessments for Adult Foster Care members.
- Complete 1–2 home visits per day.
- Attend required in-person meetings and home visits.
- Complete prior authorizations and required documentation.
- Ensure compliance with program standards and contractual requirements.
- Work independently while collaborating with an interdisciplinary team.
- Assist with program organization, expansion, and staff training as needed.
Qualifications
- Current Massachusetts Registered Nurse (RN) license required.
- Minimum of 2 years of human services experience preferred.
- Valid driver's license, reliable transportation, and a good driving record required.
- Strong communication, interpersonal, and organizational skills.
- Ability to work independently and manage priorities effectively.
- Demonstrated reliability, professionalism, and respect for diverse populations.
